    The following job responsiiltiies include, but are not limited to:


    Meet or exceed the established workload and productivity standards


    Evaluation, examination, diagnosis, and treatment of medical disorders


    Examine patients and performs initial patient work-up and assessment of patients' care and treatment; determines the need for and orders laboratory tests, X-rays, and other diagnostic tests; interprets the total medical evidence, incorporating information into an accurate diagnosis or appraisal; institutes proper treatment and patient care


    Extracts patients' body fluids / tissues; prepares patient specimens for examination, performs specialized or non-routine tests on body fuilds or tissue samples or confirms patients' test results


    Diagnoses disease, illness, disorder or injury; institutes treatment of patients; seeks advice of approrpirate Gastroenterology physicians for more serious or uncommon conditions


    Writes prescriptions, orders the administration of medications, dispenses medications and prescribed controlled substance in accordance with approved treatment protocols.  Prescribes for and treats a variety of diseases, disorders and injuries; treats patients returning for routine or directed follow-up and treatment of chronic illnesses previously documented in patients' medical records; writes routine orders on outpatients; perform medical histories


    Provides a full range of emergency services or crises interventions in the absence of the physician, including lifesaving emergency procedures in order to stabliize a patient within the scope of practice and approved protocols


    Must be able to define unknown conditions and resolve critical problems


    Documents on outpatient and inpatient records histories, physical exams, laboratory and x-ray data, assessments, diagnostic and therapeutic plans, patient progress, consultations, procedure reports, etc


    Provide consultation on referrals by Primary Care Managers


    Seeing patients to discuss findings, need for follow-up care, and timing of the next Primary Care procedure or appointment


    Records entries into patient's charts including progress notes, treatment rendered, tests or procedures ordered, notations of prescriptions or orders, and drugs dispensed or administered per approved protocols


    Provide medical advice over the telephone and enter summary of consultation into CHCS/AHLTA or other EMR to capture workload data within 72 hours


    Obtain appropriate history, physical, assessment and plan in a timely manner on each patient and develop treatment plan


    Preliminarily interpret radiographs; discriminates between normal and abnormal findings to recognize early stages of serious physical problems


    Provides consultation, teaching and coordination of all patients, their family members, and patient's Physicians regarding treatment, medical care and the need to see the Family Health Physician prior to a sedated examination by the Physician


    Provides primary care and preventitve services to healthy individuals, including guidance in common illnesses, accidents, alternative drug usage.  Evaluates the total health care needs of patients and develops plans to meet patients' needs.  Counsels and teaches patients about modification of risk factors, lifestyle changes, and medication management.  Educates and cousels patients and family members regarding procedures to be performed and treatments provided


    Participates and presents outcomes in professional conferences at local level as required


    Participates in team conference meetings


    Perform, assist, and interpret diagnostic studies within the Family Health Clinic


    Prepares medical records, reports, forms and documents.  Completes quality assessment reviews as required.  All records, forms and documents shall be complete and accurate at least 95% of the time












    Active/unrestricted license in ANY of the 50 states or U.S. Territories


    Completion of accredited Nurse Practitioner Program


    Current DEA certificate or Controlled Drug Substance number




    1 year experience as a Nurse Practitioner


    EMR capability and experience


    U.S. Citizenship (per Federal Government Guidelines)
